Phenolic of UHMW in Thin Sheets?

I think you want phenolic or even just plain old plastic laminate (AKA Formica). I dont know of anthing that will glue UHMW to BB ply plus it's rather soft. I dont think it would hold up very well for a RT top.
 
What I want is the slippery aspect of either UHMW or phenolic. Just a thin film to enhance the usefulness of the top. I thought about the steel top made by both Lee Valley and Benchdob, but the size is too limiting of those two makes

Gary, the Formica type laminate will be plenty slick, especially if you use some with the slightly rough (orange peel type finish) and then apply a little Johnson's paste wax occasionally. UHMW, (solid OR tape is TOO soft and will get MANY scratches too easily. Phenolic is NO Slicker than the Laminate I mentioned.

I'm going to go with them for a TS extension and the price I got from him was real competitive with the cast iron ones available. They'll make any size you want and he's been asking some questions on SMC of late regarding features folks would want in a standalone router table top if they were to offer some standard sizes/features.

There is something that will glue UHMW. I don't know what it is but am told such a stuff exists. It is not a hard material. Be warned, soft and subject to scratches and damage.
What probably would work better for you is UHD (ultra high density) poly. Looks the same but is hard and very machinable and durable. Again, resists glues.
