I see two of each. How are you inserting the images? Using the Insert image toolbar button or using the buttons below the post where the attachments are?

@bill kaminski Looks like I'll need a bit more info about what device you're using (Desktop? Windows/Mac? Mobile? Android/iPhone?) and what web browser (Chrome, Firefox, IE, Safari, other?) you're using?

The only way I've reproduced it is by double clicking the button to insert it into the post, if you're doing that, then it may be the cause.
