From their own report:
Hazard: The table saw’s arbor shaft can fail when used with a stacked blade set (commonly known as a “stacked dado set” and used to cut grooves). The stacked blade set can be ejected from the saw, posing a potential laceration hazard to consumers.
Does an 8" diameter dado stack impaled in someone's head, chest or shoulder count as a laceration hazard? Holy cow, what a lack of quality control. Whatever buyer/engineer sourced the arbor manufacturer probably dropped something in their shorts!
I read earlier this year that Steel City had a similar problem with one of their table saw models as well.
