We've seen more than one wooden bicycle but I think this is one of the nicer looking ones out there.

I expect from an existing bike. Or, if you knew a metal worker, a fork wouldn't be that hard for them to fabricate. You'd probably also want the head tube, bottom tube and handle bar post off an existing bike.
